How it works
The Framework
The HIS Child Transition Framework™ is a comprehensive, child-centered methodology developed by Handy Innovative Solutions to strengthen clinical decision-making, improve placement outcomes, prevent unnecessary congregate care, and support safe, stable, family- and community-based outcomes for children and adolescents with complex needs.

Rather than relying on a single service or event, the Framework provides a structured, collaborative process that guides stakeholders through comprehensive assessment, individualized planning, placement identification and matching, caregiver preparation, transition implementation, and stabilization.

Every child is different. Every situation is different. While many partners choose to utilize multiple components of the Framework as part of a comprehensive approach, each component is also available independently, allowing agencies to access the consultation that best meets the needs of the child and family, available resources, and where they are in the care and planning process.
1. Understand the Child
Comprehensive Clinical Child Profile™
Every successful plan begins with a comprehensive understanding of the child, not simply their diagnosis, behaviors, or placement history.

The Comprehensive Clinical Child Profile™ integrates an extensive review of clinical, educational, medical, legal, and child welfare records with interviews of the youth, when appropriate, family members, caregivers, providers, educators, and other key stakeholders. This information is analyzed through a clinical, trauma-informed lens to identify the child’s strengths, needs, treatment history, protective factors, barriers to stability, and individualized recommendations for treatment, placement, and transition planning.

The goal is simple: ensure everyone making decisions about the child is working from the same comprehensive understanding of who that child is and what they need to succeed.
2. Find the Right Match
Placement Coordination & Child-Centered Presentation (PCCCP™)
Identifying the right placement requires more than sending referrals or reviewing clinical records. Our structured Child & Family Matching Process helps prospective caregivers and providers develop a balanced, comprehensive understanding of the child before making placement decisions.

The process may include development of the HIS Child Snapshot™, provider and caregiver consultation, review of potential placement resources, facilitation of Child & Family Matching Meetings, and meaningful youth participation whenever appropriate. Rather than focusing solely on diagnoses, behaviors, or past challenges, prospective caregivers and providers come to understand the child’s strengths, personality, interests, hopes, support needs, and the conditions necessary for long-term success.

The goal is not simply to find an available placement. It is to identify the right match.
3. Prepare Everyone
Child-Specific Stakeholder Consultation™
Once a family, caregiver, or placement resource has been identified, thoughtful preparation becomes critical. Successful transitions require clear expectations, prepared caregivers, coordinated services, and shared accountability among everyone involved.

HIS may facilitate a Roles & Responsibilities Meeting™ to establish expectations, clarify responsibilities, identify barriers, and develop a coordinated transition plan. Depending on the child’s needs, consultation may also address caregiver preparation, child-specific training, visitation, family therapy, environmental readiness, service implementation, funding considerations, and other supports necessary for a successful transition.

Every element of the plan is considered through one lens: What needs to be in place for this child and family to succeed?
4. Support the Transition
Implementation & Stakeholder Consultation
A successful transition requires more than establishing a discharge date. As the plan moves from preparation to implementation, continued communication and accountability help ensure that identified services, supports, and responsibilities are actually put into place.

Through Child-Specific Stakeholder Consultation™, HIS may facilitate Child-Centered Treatment Team (CCTT) meetings to review progress, address barriers, clarify responsibilities, adjust recommendations or timelines when necessary, and maintain stakeholder alignment throughout the transition.
​
This structured consultation helps the team identify problems early and make informed adjustments while keeping the child’s needs at the center of decision-making.
​
The goal is to move from a transition plan on paper to a transition plan that works in practice.
5. Promote Long-Term Success
Placement Stabilization & CONSULTATION
Placement is not the finish line. The early weeks and months following a transition can be critical to building relationships, addressing emerging challenges, and strengthening long-term stability.

Depending on the child’s needs, HIS may continue to provide Child-Specific Stakeholder Consultation™, caregiver consultation, stakeholder facilitation, implementation guidance, problem-solving, and child-specific training as the child and family adjust to the new placement.
​
The intensity and duration of consultation are individualized and designed to strengthen the existing team rather than replace the responsibilities of caregivers, providers, or case managers.
​
The ultimate goal is not simply achieving placement or discharge. It is helping children and families remain stable and thrive in their communities.
Specialized Consultation When More Is Needed
Every child’s path is different. When additional clinical or system-level expertise is needed, The HIS Child Transition Framework™ includes specialized consultation services that may be incorporated at any point in the process or utilized independently based on the needs of the child, family, and system.
Virtual Residential Program™
The HIS Virtual Residential Program™ is an individualized, family-based alternative for youth with highly complex needs who may otherwise require continued residential care. Unlike standardized approaches, each VRP is designed for one youth and one family, using the Comprehensive Clinical Child Profile™ to inform the program structure, interventions, supports, and implementation.

HIS develops an individualized Virtual Residential Program Manual that translates the youth’s clinical needs into practical strategies and clearly defined expectations for caregivers, providers, and stakeholders. The program includes stakeholder training, caregiver and team coaching, structured progress documentation and reporting, and ongoing consultation to evaluate progress and respond to the evolving needs of the youth and family.
The VRP works alongside the identified family-based placement and community-based clinical, behavioral, educational, and other supports, creating a coordinated and accountable program around the youth without recreating congregate care in the community.
The goal is to provide the individualized structure, preparation, accountability, and support necessary for a youth with highly complex needs to safely and successfully live and thrive in a family and community setting.
Comprehensive Child Utilization Review™
When questions arise about treatment effectiveness, continued level of care, medical necessity, service implementation, or barriers to progress, the Comprehensive Child Utilization Review™ provides an independent, objective evaluation of the child’s treatment history and overall system response. The review examines whether services have been delivered as intended, continue to meet the child’s needs, and support the least restrictive, clinically appropriate level of care. Each review provides comprehensive findings and practical recommendations to inform treatment planning, funding decisions, permanency planning, and future service needs.

The goal is to give stakeholders an objective understanding of what has occurred, what is working, what may need to change, and what should happen next.
Adult Attachment Inventory (AAI)
The Adult Attachment Inventory provides specialized clinical insight into how a caregiver’s attachment history may influence caregiving relationships, emotional regulation, responses to stress, and interactions with a child. The assessment may be particularly valuable when considering caregiver selection, reunification, adoption, permanency planning, or preparing a caregiver to support a child with significant trauma or attachment-related needs. Findings and individualized recommendations help stakeholders understand relational strengths, potential challenges, and areas where additional support may strengthen long-term family success.

The goal is to better understand caregiver-child relational dynamics so that placement, permanency, and support decisions are informed by what both the child and caregiver need to succeed together.
